This happens to me alot when fighting near the gate of my outpost, alot of the enemies fall through the floor also when knocked unconsious. Talking about outpost gates, if my gate is locked and my blokes are inside my outpost, sometimes if i ask them to heal someone or perform an action they unlock ...
