Actually, jumping from a roof should be an option, too, though one that should definitely hurt (moderated by Toughness), as well as leave your character to get back to their feet. I ran into this situation having my character escaped from a cage on the roof wall...it was basically teleportation sin...

I think what I'm thinking is that the overall risk in Kenshi is very instantaneous. You go through very long periods where nothing happens with brief flashes of excitement. You know, I did think of that too. A backpack could definitely be struck somewhere other than the bottom, but how would we show that on the inventory GUI? I can understand the desire for realism, but we might as well make items lower in your backpack invisible because you can't see them under the stuff on top of it. Not totally true, swords come with a hilt that can be used to batter up an opponent like a metal fist.As for lethal vs not lethal, when you use a metal sword in combat it is always lethal.
There could be some kicking, punching, use of the hilt and take-downs with momentum for non lethal combat.

Considering the historic use of elephants, it should be possible to tame a leviathan and mount some kind of platform on them with harpoons, but the leviathan doesn't have to be invincible.
