I know is late, but as i said in another post i like to see people suffer playing kenshi. I don't know if it's difficult to implement but a 2x death chance with only one save file is going to be the hell of the pu**y streamers.
I don't think you're very clear with what you want, but I believe you're talking about a new game option where you turn off the ability to save and load normally. You cannot walk back bad decisions or bad outcomes. You just continue. Technically you can do this by simply never reloading - but something like this takes away the temptation.
I suppose it would also be the case that you can't mess with the sliders except at the start. No turning down hunger for a bit, chance of death, etc.
While I'd welcome such a feature, I doubt we're going to get it at this point. They're pushing towards 1.0 and adding new things is a distraction.

I really want!
This is also called Hardcore mode in other games.
In Mount&Blade you have this same option on a new game. ("Realistic! No saving without quitting!")
There are save-scrum exploits in this game, which would be fixed if you couldn't roll-back.

The thing is this is a single player game. if you want to go iron man then go iron man. If not then don't. The only person it effects is you. I really see it as a vanity issue. "oh I play Iron man mode, if you don't your just a wuss!" just seems stupid in a single player game where your choices only effect you. If you can't trust yourself to play iron man without cheating, well that's your issue, who cares. With mods and game toughness sliders and importing, iron man mode would really be meaningless.

The thing is about the knowledge of either being able to reload or not. I'll give an in-game example from different games. Someone mentioned Mount&Blade - you play a completely different game if you can't make cupious amount of saves to revert bad decisions. You're more cautious, more realistic. In Tales of Maj'Eyeal, you have a difficulty option of Adventure or Roguelike, where Adventure gives you 5 lives and roguelike only 1. I used to play adventure and died the whole run felt dead, so I switched to roguelike. It has randomly generated enemies that sometimes can get so overpowered and insta-kill you. Pretty unfair but I'd still rather play that than Adventure again.
In Kenshi, would you go into spider infested ruins if you couldn't reload? Not without a squad or a hyper-stealth under the cover of night character. Becase death would be an ever-looming threat instead of an inconvenience of having to reload a save-file. "It's a single-player game" is moot, as only single-player games can practically have an ironman mod. Granted, some multiplayer games exist but even then there's never a save/load system in any multiplayer game.
"You can do that by deciding not to reload", well but it doesn't feel the same, and playing a game is all about the feel of it. There's not only temptation, but the knowledge of being able to reload a bad decision makes you make bad decisions more often. The whole playstyle changes this way. So I can understand iron-man mod as a feature request. Granted, though, that playing iron-man in a game where you can get stuck in a wall or somewhere impassable or when controls sometimes lag out while enemy NPCs don't (I am looking at you, Leviathan Coast) isn't the greatest idea.
