I have just been in a discussion on steam because I thought the Feature Requests were there.
I feel that the toughness ability is good and all but I would like an attack mode that disregards blocking/defence for damage for damage. Like for example when an enemy is in the motion of an attack. Your character, instead of blocking, also attacks too. Trading damage for damage, eye for and eye. Of course this also builds up toughness and attack. If your character is already a badass, a character you've invested time on wholeheartly be able to have this attack mode, he'll be able to help out your squad mates, your weaker ones. Imagine a character that has high stats and good gear you've provided for him. Be able to take out enemies quicker, for a risk of damage, but you already have good stats and good gear. Fights can last longer or shorter. Enemies are strong anyway and even these enemies can have these certain attack modes.
Attack modes
An "attack only" option seems like a good idea in large crowds where you're having a hard time getting an attack off before being stuck blocking, or stumbling, but this would actually do terrible things to balance, and there is already ingame measures to be taken to prevent you from having this struggle.
Combat in Kenshi is based on an attack turn. When it's someones turn to attack, the opposition roles it's defenses against the attack. Depending on the attack rate (now hidden value), determines who's turn it is to attack. A higher attack rate means you get your turn more often then the opponent. If you circumvent turns so it's always your turn, you throw out too many important variables that order combat balance.
You would overwhelm your enemy, continuously attacking. The faster attack speed would win and two handed weapons would be entirely useless against one handed weapons held by the player, unless the opponent also had this toggled. That 60's in stats inquisitor paladin would be mowed down by a newbie with a ninja blade, regardless of stats. NPC's do not use toggles like Block Only, but if they did, adding this toggle to NPC's would mean your characters would also be trapped doing nothing but blocking/dodging until they were defeated, or you toggled this on for them as well (forced micromangement on a very painful and annoying level), which would just cause the fight to end rather rapidly, and negate half the combat stats favoring only equipment. Having a small newbie squad with "attack only" enabled would eliminate any and all oppositions before they could ever get a chance to attack. I don't think adding a toggle like this would be a good thing.

50 toughness = 0% dmg modifier against you.
100 toughness = -50% dmg taken.

If counter-attacks were put in this would work. They would know you're overly aggressive and smack you right in your face on your second attack.
