It would be cool if you could get two of your people to spar but to make sure this isn't ridiculous and you can get super tough without ever leaving your starting town you can only get to the skill of the best person currently sparring. It could be made so your skills progress slower than in regular combat but only your limbs are damaged or something, it would also be good for training up a medic as they can heal after the fight.
surgery would also be a neat addition if say once someone reached 50 in the medical skill and you research the correct tools you could surgically remove body parts from slaves or other prisoners and store them in some kind of freezer (powered of course) and use them as a cheap alternative to bionics although you could have a small chance of the new host's body rejecting the transplant and it needing to be removed before it kills them. it could also be complicated if you need to get a fairly similar leg so a six foot man cant have a four foot woman's leg.
Please let me know your thoughts and if you would want the developers to implement either of these in the future.
Sparring & surgery. My random Ideas
I really like the sparring idea. The surgery idea however seems far fetched. In reality, such a procedure requires immense knowledge, skill, time, and is rarely performed.

Now how about damage? Will they use actual weapons and cause life threatening injuries, or will they use wooden weapons to only cause bruising and soreness? I think that choice should be made by the player. Like you said, it could also help medics hone their skills.

-Sparring participant have the real threat of death.
-They bleed out, their health bars take "red" damage.
-Requires medical attention after each spar to prevent death.
-If a novice fights a master, the novice has a chance of losing limbs and stuff, or maybe even instant death.

-Faster recovery times than using real weapons.I've always thought it would be nice to just have an order for attacking squad-mates. Adding wooden/training weapons would allow you to reduce the amount of injury done, and maybe themselves impose an experience penalty. I can agree that it would shift the balance of the game a bit, but perhaps that could be mitigated with some kind of morale score so that the more you pit your units against each other, the less they feel like fighting in general.
You can still train without really leaving the starting town, though, by simply dragging the unconscious body of an bandit into your house/outpost in town and healing them. Every time they get up, your unit(s) get more training.
The surgery idea seems a bit much, sounds more like what a mod should be able to do eventually (reminds me of the Cannibalism mod for Morrowind). I don't think the medical science in the Kenshi world is quite up to the task of attaching someone else's body parts to a patient. Though, perhaps when limb-severing is implemented we'll be able to pick up the hacked off arms and legs and use them as an easy food source. Seems like that's exactly what the Cannibals would want to do.
