Random Crashes, ran out of memory.

Tried disabling shadows, no far towns and overall lowered graphics and render distance.

Looks like a crash in the terrain engine. Would be worth trying a clean reinstall, might be you have some corrupt files. Make sure to back up any save/mods before uninstalling.
