Smithing crafting error

hey there,
I'm not sure if this is a bug or not but i cant seem to get round it.
Basically, in game i've got this setup with everything working except smithing. Each time i get a guy to create a weapon it always disappears when it is finished, the output just disappears. I havent found anything else wrong apart from this one thing, so im wondering if i am doing something wrong.
I have all the right and necessary ingredients but each time exactly the same thing happens and ive checked the inventory of the person and nothing. Apart from this the game is great and ive enjoyed every minute, thankyou.
