[v1.0.40] Map Errors

Chatting with some folks on discord and we came across some errors in the map, looks like they were probably unintentional accidents. Screenshots attached, with coordinates and map shown.

Gash found in the Outlands, similar to the one from up in the Cannibal Plains along time back.
