I have always enjoyed designing a character, building them up, joining them on their experiences, and more or less developing a personality and story for them. These kinds of thoughts also are entwined with the thought of that character having items that are, more or less, specific to them. Not that they are specifically special items, but that the character has used them for so long and is so comfortable with them, that that specific item works better in their hands.
I find the idea exciting of having, for example, a Katana that eventually gains bonuses on attack, defense, speed, special abilities, what have you, the longer the character uses it. Now if that weapon (or item), is dropped and picked up by any other character, it would work as just a normal Katana.
OR, it could be possible that this item, again Katana, eventually gains a "personality" from all of the use and can impart it's full benefits onto its original wielder, and only parts of its benefits to anybody else, or possibly causing negative effects on anybody else wielding it.
I feel that would go along with real life situations, as in how an individual is an excellent shot with a pistol and knows almost all pistols in and out, but there is that specific pistol that is theirs; they know the weight, the grip of the handle, the recoil, how the bullet leaves the barrel, etc, and can do amazing things with this weapon. Now this could be argued that the person is just experienced with his weapon he knows how to use it, but ultimately, that is what I am referring to if somebody where to look at it in that sense; any other pistol they are great with, but that one is theirs.
Maybe this could be satisfied with a special material in attempts to only allow limited special items in the game.

I could see something like a 10% familiarity bonus to weapons, that could be an interesting mechanic that offset the decision to upgrade from a decent weapon to one with only marginally higher stats. As for renaming weapons and such, I'm not sure if that is in the pipeline or not to be honest, but it would be neat if we could change around names and rewrite descriptions on specific weapons.

This is a very good idea and not something that I've seen before. Seems like it'd be fairly simple to implement too?

I second this motion heavily.
Also I do hope Chris implements the ability to rename "HomeMade" on player made items. If I'm a Merchant I'd like to have a future where my products would be more infamous or famous than the ingame manufacturers such as Edgewalker, or Metal Purity, etc. Having the ability to make your own franchise would be great.
